Univerity of Wisconsin System Administration (UWSA) provides the reporting structure that each campus uses. The various campus environmental health & safety departments provide the forms to the claimant and acts as a point of contact for the claim. The campus reviews the injury with the employee and/or the supervisor (and completes an investigation if warranted) and then completes a safety manager’s review form, which is submitted to UWSA, as part of the process. UWSA records and compiles the injury logs and distributes them to each campus and they are displayed or provided to employees as required by OSHA.
