A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :
The Workplace Safety and Insurance Act (section 21(1)) requires the employer to notify the Workplace Safety and Insurance Board within three days after learning of an incident to a worker if it necessitates health care or lost time. York University submits claims to the Workplace Safety and Insurance Board monthly. Incidents, hazards, occupational illnesses or injuries must be reported to the injured employee's Management Supervisor, who will then report it to HSEWB. Workplace Incident Report (WIR) is to be submitted to wir@yorku.ca. Management Supervisors - Provides assistance to the employee - Conducts the incident investigation - Complete mandatory online Incident - Investigation Training. It is recommended that this training be completed once every three years. - Provides corrective action to eliminate or reduce hazards - Completes the Workplace Incident Report (WIR) within 24 hours and sends a copy to Employee Well-Being (EWB). - Notifys the appropriate JHSC worker member - Provides the employee with a WSIB Kit (cover letter and Functional Abilities Form) as needed - Refer to the Incident Investigation Program (PDF) for details."
