Has the institution conducted a survey or other evaluation that allows for anonymous feedback to measure employee satisfaction and engagement during the previous three years?:
Yes

Percentage of employees (staff and faculty) assessed, directly or by representative sample (0-100):
12

A brief description of the institution’s methodology for evaluating employee satisfaction and engagement:
Facilities Planning and Management within the Division of Finance and Administrative Affairs completed an engagement and employee satisfaction survey in the fall of 2018. Survey questions were designed to measure the satisfaction with the work environment, relationships with co-workers, expectations and resources, training and supervisors and management. Open-ended responses have been grouped together into thematic categories and summarized. Upon completion of the survey, leadership presented all FPM employees with the results and have begun to form Engagement Teams to address issues found in the survey. As of summer 2019, Engagement Teams are finalizing recommendations and programmatic changes. The last formal campus-wide employee satisfaction survey took place in 2012, which is too old to qualify for this credit.

A brief description of the mechanism(s) by which the institution addresses issues raised by the evaluation (including examples from the previous three years):
FPM leadership convened teams to address the primary issues raised in the engagement survey. The teams consist of employees who volunteer to develop programs and outreach to address the primary issues. An example of an outcome from the survey is an increased focus on onboarding through new employee orientation.
