Our assessment was developed based on three areas
a) Knowledge
b) Belief/attitude
c) Practice

We administered a pilot survey to a class of undergraduate students during the fall 2014 and refinements were made based on that pilot, Another test survey will be conducted in early 2015, and again refinements will be made on the basis of the result of and feedback on that test survey.

A brief description of how the assessment(s) were administered:

We hope to administer a broader survey during the 2015-16 academic year to a defined cohort of first year students, with follow-up with the same cohort of students in three years.

The survey was generated online and was sent to students' NSID
