Work-related injuries and illnesses are reported using Accident and Injury Report forms and Worker's Compensation First Report of Injury or Illness form. The OSHA Form 300 Log of Work-Related Injuries and Illnesses is used to record and classify work-related injuries and illnessess. The OSHA Form 300A, which summarizes the total injuries and illnesses for the year in each category is posted annually.
