Has the institution conducted a survey or other evaluation that allows for anonymous feedback to measure employee satisfaction and engagement during the previous three years?:

Percentage of employees assessed, directly or by representative sample:

A brief description of the institution’s methodology for evaluating employee satisfaction and engagement:
The survey was administered by a third party, Perceptyx, and focused on the areas of: engagement, diversity and inclusion, teamwork & collaboration, empowerment, well-being, manager relationship, recognition and other important indicators of a healthy and robust work environment. The online survey was distributed to our faculty and staff via email invitation directly from Perceptyx (customersupport@perceptyx.com) with details about how to complete the survey. Vendor Perceptyx compiled all survey responses into an aggregate database. Aggregate reports are available to leadership and shared with employees by work area. Leaders do not receive results for their team if there are fewer than 5 people who responded on their teams.

A brief description of the mechanism(s) by which the institution addresses issues raised by the evaluation:
Action plans created by leaders were based on unit results, then implemented. Updates on action planning were submitted to their supervisors.
