A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :

Employees/Supervisors complete an online First Report of Injury form, which is transmitted to our Third Party Administrator (TPA) for investigation, determination of compensability, and distribution of benefits. The TPA’s program analyzes and calculates the data daily, which is then queried by the University as needed for OSHA reporting.
