Here at the University of Arizona, we do not formally follow an OHSMS program such as ISO 45001 or ANSI Z10 standards. We incorporate some of the principles of the systems in our safety programs such as Plan, Do, Check, Act, and hazard identification but it is not a formal management system. We stress OSHA compliance and hopefully, best practice compliance where there are new health and safety recommendations from such organizations a ACGIH or ASSP."

A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :

"UA-RMS has implemented the “Origami” system for reporting injuries, incidents, and claims. If a claim meets the OSHA reporting requirement, this system will track all OSHA recorded incidents by manually entering as such along with the number of calendar days the injured worker is off or is at a light duty assignment. The OSHA Recordkeeping forms 300 log and 300A summary are generated by February 1 of the calendar year and posted. These forms must be maintained at the worksite for at least five years. Each February through April, employers must post a summary of the injuries and illnesses recorded the previous year. OSHA records are maintained in the UA Risk Management department. OSHA defines a recordable injury or illness as: - Any work-related fatality. - Any work-related injury or illness that results in loss of consciousness, days away from work, restricted work, or transfer to another job. - Any work-related injury or illness requiring medical treatment beyond first aid. - Any work-related diagnosed case of cancer, chronic irreversible diseases, fractured or cracked bones or teeth, and punctured eardrums. - There are also special recording criteria for work-related cases involving: needlesticks and sharps injuries; medical removal; hearing loss; and tuberculosis."
