A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :
All work incidents occurring at UQAM are declared using the work incidents declaration form. This form is available via the website of the Organizational Development Service (ODS): https://sdo.uqam.ca/sante-securite-au-travail/declaration-daccident-de-travail/ and the human resources site : https://www.rhu.uqam.ca/index.aspx?id=p269.
This form is then sent automatically to the relevant stakeholders: the prevention team at the Organizational Development Service, the compensation/disability team at the Global Compensation Service, the managers and the related union association.
This is how we get a number of incidents per year for the establishment.
This form is then sent automatically to the parties concerned: the prevention team at ODS, the compensation/disability team at the Global Compensation Servic, the managers and the related union association.
