Your Plan for Health (YP4H) is The Ohio State University's preventive approach for providing faculty, staff, and their families with the tools and resources that will empower them to pursue a life of health and wellness.
YP4H services include Biometric Health Screenings, Personal Health Assessments, health-related financial incentives, Personal Health Coaching, Care Coordination for chronic conditions, and The Ohio State Employee Assistance Program, Educational Programming, and much more.
