Our program is built upon the requirements under Oregon OSHA - General Occupation Safety and Health Rules, Oregon DEQ, the EPA, and Portland Fire Bureau (to name a few). Additionally, EHS supports research safety on campus, including Radiation Safety Committee, Institutional Biosafety Committee, the Institutional Animal Care and Use Committee, and the Laser Safety Committee, which all have their own guidance documents and standards through other regulatory agencies.

A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :
Portland State historically used a paper form titled "PSU Injury Report" that was filled out and sent to HR. This year we converted the paper form into an electronic "e-form" in OnBase. You can now access the form online through this link (accessible to PSU community members only): http://bit.ly/PSU-InjuryReport .
Once an employee fills out the form from the link above, it enters OnBase and from there, our Leaves Team is able to track it, send notifications, and keep totals on the number of forms submitted.
EHS partners with HR on matters of workplace accidents and injuries. Any recordable injury is reported to HR , either directly through their online report injury form, or via their supervisor submitting an injury report form via fax/email. HR is the primary clearing house of all recordable workplace injuries. EHS assists HR in conducting accident/injury follow-up analysis (investigations) and offers corrective actions, such as additional training, expanded use of personal protective equipment, Standard Operating Procedure creation, or programmatic changes. The numbers you supplied look accurate for the number of recordable injuries reported for FY 2019.
