Part 1. Health and safety management system

Does the institution have an occupational health and safety management system (OHSMS)?:
Yes

Does the system use a nationally or internationally recognized standard or guideline?:
No

The nationally or internationally recognized OHSMS standard or guideline used:
---

If no, provide:

A brief description of the key components of the custom OHSMS:
Penn State uses a customized approach that has many elements of a standardized OHSMS, but it has not formalized its approach in accordance with recognized standards such as ISO 45001 or OHSAS 18001. Elements that currently exist include leadership commitment, employee involvement, hazard identification mechanisms, hazard control methods, employee training, etc.

Part 2. Incidents per FTE employee

Annual number of recordable incidents of work-related injury or ill health:
192

Full-time equivalent of employees:
16,385.76

If the institution wishes to report on other on-site workers, provide:

Full-time equivalent of workers who are not employees, but whose work and/or workplace is controlled by the institution:
---

A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :
OSHA recordable cases are tracked by PSU Environmental Health & Safety (EHS) in collaboration with the PSU Occupational Health group. Occupational Health determines recordability for each work-related injury or illness in accordance with OSHA requirements. EHS used this recordability information to calculate incidence rates and conduct additional data analysis.

Annual number of recordable incidents of work-related injury or ill health per 100 FTE employees:
1.17

Data source(s) and notes about the submission:
Number of incidents reported in Part II of this credit is based on information supplied/tracked by the Occupational Health and Safety Manager in the Environmental Health and Safety Department at Penn State. Their calculated incident rate, 1.7/100 employees, is based upon actual work hours obtained from the Office of Human Resources.
Therefore the FTE of employees reported in Part II has been adjusted to agree with the numbers used in OHS's calculations. OHS's work hours include student work hours, so the FTE of employees is higher than that reported in PRE-5. In PRE-5 we exclude student employees from the employee FTE, to avoid double-counting these individuals as both students and employees.
