Part 2. Incidents per FTE employee
Full-time equivalent of employees:
If the institution wishes to report on other on-site workers, provide:
A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :
All injury and illnesses included on IU's OSHA 300A document are included as an individual recordable occurance. As OSHA 300A is reported on a calendar year, the numbers represented here are for Jan 1, 2022 through Dec 31, 2022, which differes from IU's reporting timframe for the rest of this STARS report. Bloomington's numbers represent the reporting for the Bloomington Campus as well as the auxilery campus of Bradford Woods, which is included in Bloomington's reporting boundary.
Occupational Injury and Illness Investigation Program (IU Policy PS-EHS-07) https://policies.iu.edu/policies/ps-ehs-07-occupational-health-safety/index.html Ensures that IUEHS is informed of employee injuries and illnesses for the purpose of incident investigation and prevention and to establish a mechanism for compliance with the Occupational Safety and Health Administration (OSHA) rule for reporting work-related fatalities and severe injuries/illnesses (29 CFR 1904.39)
On the Job Injury Procedures are reported online through IU Human Resources: https://hr.iu.edu/workers/index.html#loaded In accordance with IU Policy_HR-11-30_Injury On The Job https://policies.iu.edu/policies/hr-11-30-injury-job/index.html

Data source(s) and notes about the submission:
Environmental Health and Safety (EHS) reports workplace injuries and occupational disease cases to the Occupational Safety and Health Administration (OSHA) as numbers of incidents out of total employees (who worked for the University for any length of time, and for any number of hours per week, full or part-time, including student workers). Therefore, the number of "employees" of IUPUI, for consistency with EHS and OSHA is 19,937 in the performance year. The above figures represent incidents of an annual average of 19,937 employees. This is the figure we have used rather than the Pre-5 figure, which comes from the IU Faculty + Staff Headcount (excluding student hourly). Data provided is from calendar year 2022, while full-time equivalent data in Pre-5 is from FY22 (July 1 2021-June 30 2022). http://hr.iu.edu/workers/ https://policies.iu.edu/policies/hr-11-30-injury-job/index.html https://ehs.iupui.edu/index.html
