A brief description of the institution's indoor air quality plan, policy, and/or practices:
The compaint is reported to Facility Services. Facility Services maintains a record of all reported IAQ specific issues. A work request is then ordered and Facility Services notifies David Cox by email or phone. Incident is investigated and report is made. Plan of Action is then determined by Safety Manager, David Cox.
