New hires must complete the Occupational Health Services form. Your supervisor can assist with completing the form by reviewing the Supervisor Guidance for Occupational Health Services. The completed form must be returned to Human Resources. Human Resources will forward the completed form to EHRS. To schedule an appointment with the occupational health physician, the employee must complete the Request for Annual Health Review form. The form must be sent back to EHRS (ehrs@drexel.edu) who will schedule the appointment with the occupational service provider. An employee’s classification will determine what services are provided.
In addition to the initial health services, employees will undergo an annual health review by university’s occupational healthcare provider. The employees will be notified by EHRS when the annual health review is due.
