A brief description of when and why the workplace health and safety baseline was adopted (e.g. in sustainability plans and policies or in the context of other reporting obligations):
Environmental Health & Safety (EHS) reviews all incidents with the objective of preventing another injury. Currently we closely monitor slip/trips and falls and activities targeted to prevent them. This is now one of the major injuries in most industries along with ergonomic injuries.

A brief description of the institution’s workplace health and safety initiatives, including how workers are engaged in monitoring and advising on health and safety programs:
(1). We have a Campus Safety Committee that meets 8 times a year to adress campus-wide safety topic. (2) Currently CSU is a member of a consortium of university EHS professional who have developed online H&S training for universities and their research laboratories. CSUEB is represented on the CSU committee working to bring some of these online classes to the CSU campuses through Skillport.

The specific fact book is Faculty and Staff Headcount and FTE. http://www.csueastbay.edu/ir/files/html/University.Employees.8-1.html Weighted Campus User in performance year for PA 14 differs from IC 3, because IC 3 indicates the current year as the performance year (AY18/19) and PA 14 indicates performance year for calendar year 2017 in alignment with most current dataset available when this credit criteria was completed.
