Occupational injuries and illness are reported to Human Resources on a standardized injury and illness reporting form. This form is filled out jointly by the affected employee and their supervisor. The supervisor is responsible for filing the report promptly with Human Resources who reports it within twenty–four hours to our workers’ compensation insurance carrier. Root cause investigation as well as any medical attention completed and/or coordinated through Human Resources.

Human Resources determines recordability for each work-related injury or illness in accordance with OSHA requirements. The injury log is reviewed and certified at the end of each calendar year prior to posting and reporting.
